What exactly should i feel in the blancs to broadcast my programm on Internet with the help of listen2myradio.com?

I have an account and a station on listen2myradio.com.

What exactly should i fill?

server- my host with http or without
port - 8000?
name- ...............?
password- the password which i login on listen2myradio.com
genre- ........?
