When i have the level slider on 0% and eg put it up to 10% it goes to 100% Volume through my headphones. i have to turn the volume knob to the top and then to 10% again in order for the volume to work correctly.
This also happens when you have it set to anything lower than 100% pause the song, then play it it goes up to 100% volume.

diagnosed the issue, it happens on headphones, not on speakers
